I have some of the 'dear doctor' postcards, but I can't translate this one well enough to say if it is one of the original ones or not.

I had never heard of a Dear Doctor postcard before now, so did a little research. Interesting! I hope you don't mind, I was curious, so also ran it through Copilot, and came up with

But... it felt the "inferno" is a playful thing, like a "hellish, tropical place," a joke about the climate there. Also, I learned that My Esteemed Doctor was often used for women with professional status, not necessarily in a medical sense. Like a lawyer, a manager, as a title of respect.
